MyHF, a smartphone application which you can use to monitor your heart failure.

Welcome to MyHF, a smartphone application provided by your Doctor which you can use to monitor your heart failure. With MyHF you can regularly record your medical parameters, such as weight, blood pressure, heart rate, and quality of life assessed by means of a short questionnaire. The history of you parameters is displayed in graphs to help you follow the course of your disease and to show your Doctor at appointments how your parameters are changing. MyHF also offers practical tips on learning more about heart failure and its management, through two main topics: diet and exercise. Finally, you can also use MyHF as a daily treatment reminder: enter your current treatment and activate the push notification, to remind you to take your medication at the specified times.
